| Two PHF Contribution - Recognition Transfer forms into one document |

We are at the dawn of becoming a far more efficient organization and part of that program is the rationalization of our two PHF Contribution/Recognition Transfer forms into one document. No more will you wonder which form to use as we will now have only one form. You will use one side (123-EN) to make a contribution and allocate any resulting points and use the other side (102-EN) if you are only wishing to transfer points without making a contribution. The forms have been reivised to make them more 'user friendly' and I'm sure you will agree that this is a great step forward. Many of our more technologically advanced colleagues will lament the fact that we still have a 'complete and print' document that cannot be saved when completed. I agree that this would be a far more efficient way to manage the process and I will be making representation to my TRF colleagues in that regard. In the meantime if you want to transmit electronically you will still have to print, scan and save. You will also notice that the contribution page (123-EN) that mention of American Express, Discover Card and JCB credit cards is included but we are trying to have the SPPO form specially adapted to exclude these. We will also have the Chicago postal address removed and replaced with the Parramatta address. In the meantime, welcome to this new way of doing things. I think you'll like it. |
